Home Support
Prestwood Home Support is a Registered Domiciliary Care Agency. We provide specialist support for people with mental health problems and/or learning disabilities, enabling people to maintain their independence within their own homes.

What you can expect from us

Prestwood Home Support are people who can help you to keep living in your own. We try to make sure you are happy and successful, and as independent as you possibly can be.

Prestwood Home Support is committed to providing an excellent service to you.

To achieve this:

  • Provide qualified home carers to meet the needs of the service user with complex and specific requirements.
  • Work closely with service users, carers, relatives and friends.
  • Ensure the service is user focused and inclusive of ethnic, religious or cultural requirements.
  • Help service users, where possible, to stay at home, rather than move into residential care.
  • Consult with service users in relation to planning and future delivery of the service.
  • Work in partnership with other services and organisations to ensure service users needs are met.
  • Aim, where possible, to deliver services at times convenient to our service users.
  • Ensure all services are delivered in accordance with our Quality Assurance Policy and Procedures.
  • Provide each service user with a Service User Plan detailing their services and maintain accurate and up to date records in the service users home.

Services We Can Provide

  • Getting up or going to bed.
  • Assistance with personal hygiene.
  • Shopping, bill paying or banking.
  • Prescription collection.
  • Escorting duties, whether to the shops, banks, doctors, hospitals etc.
  • Sit in service.
  • Sleep in or waking night service.
  • Supervision (but not administration) of medication.

How services will be provided

Services can be provided for up to 7 days per week and are available 24 hours per day; this includes bank holidays and weekends.

When the service commences a manager will visit in order to establish a plan as to how the service will be provided. A risk assessment and health and safety check will also be made.

The plan will be discussed with the service user and agreement confirmed with their signature.

There will be a continuous review of needs and the plan can be changed either temporarily or permanently.

Service users can make contact by telephone or email. Any changed to the service plan will be negotiated and agreed with service users.
